Bulletin number: SB116
Date issued: August 26, 2009
Machine models: CUBIX Machines through P11636
To our Valued Customer:
This safety bulletin is part of an ongoing effort to provide quality service and support for all Alliance Machine Systems International, LLC products. If you need assistance or more information, please call Gary Kneale, Field Service Manager, at 509-535-0356 and select option two.
Description of Issue
On certain Cubix machines, there is no time delay relay on the operator access door to prevent access while the machine is still moving. Opening the door does cause the machine to initiate a stop but momentum could cause belts and other components to still be moving while an operator or maintenance personnel are attempting entry into the machine.
Discussion
Alliance has designed a redundant control circuit with a time delay relay that latches the access door closed until all components inside the machine have had time to stop moving. The circuit is comprised of a dual wire E-stop circuit with safety monitoring relay and a second safety monitoring, time delay relay connected to the door latch.
Recommended Action

Alliance recommends that you take the following actions within 10 days after receiving this safety bulletin.
- Verify that a “Hazard Area, Follow lockout procedure prior to entering.” warning decal is present on the door or immediately adjacent to it. If no decal is present please call 509-535-0356 and select Option two to request additional decals. Retrain your operators to verify that all motion has stopped prior to opening the access door and that they follow proper lockout procedures prior to entry.
- Alternatively, the time delay retrofit kit should be installed. This kit contains all components and wiring necessary to prevent the access door from being opened until the motion inside the machine has had time to stop. Again, please call 509-535-0356 and select Option two to request additional information.
