Description of Issue

Alliance has become aware of an operator injury on an ACCUSTAK® that was the result of an operator entering the lift area hazard zone without using proper lockout procedures. An ACCUSTAK operator entered the lift area while the machine was running. The operator was trying to straighten a bundle or a stack when he caught his hand on an in-running nip point created by the movement of the lift conveyor belt.

Discussion

The ACCUSTAK is a safe machine when the safety guidelines for the machine are followed. This includes locking out the machine before entering the hazard area and following the safety procedures included in your operator/maintenance manual.
When bundle/stack problems occur, they should be moved out of the machine before the operator tries to correct the issue. If this is not possible, the operator should lockout the machine before entering the hazard area.

Recommended Action

Alliance recommends that you review the safety chapter of your operator/maintenance manual with all ACCUSTAK operators as soon as possible after receiving this safety bulletin.  Please pay particular attention to the Hazard Area section and the High Hazard Moving Parts section. Alliance also recommends that you verify that the lift exit awareness barriers are securely installed and that hazard area decals are/have been applied to the barrier.
